Steps to Making Appointment Online:

• Note: we accept most insurances, but not United Healthcare or Medicaid
• Click the Book Online Here Button Below
• Pick whether you are a New Patient or Existing Patient 
• Agree to Terms
• Select Reason for Visit
• Next select the doctor you want to see and office you want to see them (we have 2 office one in Clermont and Leesburg)
• Choose time that is open
• Next fill out all of the patient information: Name, address ect.
• Scan insurance cards if you can, if not make sure you bring them to appointment
• It is very important that you give us the best number to receive confirmation call/text and email address to communicate regarding your visit 
• If at any point you get frustrated feel free to call the office and someone from our office can help you make the appointment that way
• We have someone on staff full time that verifies all patient's insurance, your information entered needs to accurate so we can let you know what to expect you might owe during the day of your office visit.  So if you can upload the actual cards that helps with accuracy.
